Offers statewide dental program based on income, age and disability criteria. Referral agency matches clients with dentists to receive comprehensive dental services.
Intake procedure: Application required; not everyone will be selected for assistance, there is a very long waiting list.
Fees: None
Eligibility: Must meet income guidelines, be uninsured, and be 60 years of age or older OR have developmental disabilities OR be a veteran.
Documents required: Must provide proof of income for everyone in the home, DD214 for veterans, Social Security Determination letter for those receiving SSI.
