Permanent Clinic.PrimaryPlus accepts most insurance, Medicare, all Kentucky Medicaid plans, OH Medicaid (CareSource & Molina) and offers a sliding fee scale for those that have NO insurance and/or are underinsured. The Sliding Fee Scale can provide a discount of up to 75% based on application approval.
