Dental Evaluation & Extraction
Persons who qualify for Dental Care or Eye Care:
Have needs that are not being met by other care givers.
Can not have Medicaid, Medicare or any other medical insurance.
Have household income that falls below 125% of the federal poverty guidelines.
Proof of income and pre registration are required except in emergency situations.
Cannot have insurance that covers Dental or Vision care.
Work Hour
9:30 am - 12:30 pm Monday through Friday
